Position: Program Coordinator/Director
Position Summary: The Program Coordinator/Director will be responsible for implementing, coordinating, and participating in the delivery of therapeutic activity program services, supervision of Activity Assistants and for basic facility oversight. The Program Coordinator/Director has the authority and responsibility for the management of activities and direction/supervision of program assistants to ensure that activities and services are provided in accordance with established program policies. The Program Coordinator/Director reports directly to the Executive Director.
PRINCIPAL DUTIES AND RESPONSIBILITES:
1) Prepare for Person Centered planning meetings by keeping abreast of the Person Centered Chart and providing data for the Executive Director. Participate in Plan of Care meetings when possible, or at least by preparing written info for the purpose of contributing to person centered service planning and supporting participant-caregiver expression of individual goals.
2) Design and implement a therapeutic activity program/schedule that incorporates activities in a social setting that are:
Cognitive/Physical/Psychosocial
The purposes of activities shall include promotion of self-expression through intentional activities reflective of life history and interests. They shall meet core psychological needs, e.g. comfort, inclusion, attachment, unconditional acceptance and occupation. As appropriate, they shall encourage new skills and mastery of adaptive devises. There shall be intent to build self-esteem by providing positive reinforcement, diverse care plans, and mixing the familiar with “something special”.
3) Facilitate a team approach to providing services by soliciting ideas from staff and delegating tasks to the staff and volunteers. Provide direct training and supervision to Program Assistants.
4) Collaborate with the Executive Director in orienting Program Assistants and volunteers. Assist with recruitment, record keeping and recognition of volunteers.
5) Open the Center each morning and be present to assist with maintaining adequate ratio.
6) Provide direct care by assisting with activities of daily living, personal care, ambulation or nutritional needs, on an as needed basis, especially to determine appropriate cuing /guidance.
7) Attend trainings, meetings, as needed, to gain knowledge to improve the center’s activity program services and safety.
8)Maintain procedures and forms (documentation) for activity program services, volunteers program, and activity assessments.
9) Routinely use computer, printer, FAX, phone and answering machine.
10) Regularly lifts 10lbs. frequently assist adults up to 150 lbs., occasionally (with partner assistance) assist adults up to 300 lbs.
11) Consider and appropriately prepare for exposure to: A) blood borne pathogens B) outside weather conditions.
12) Duties otherwise assigned by the Executive Director to enhance service provision and safety, including the ability to work overtime if needed.
Qualifications:
Must be at least 18 years of age. Have completed a minimum of two years of formal post-secondary education from an institution accredited by an accrediting agency recognized by the United States Department of Education (including colleges, universities, technical institutes and correspondence schools) or shall have a high school diploma or the equivalent and a combination of a minimum of five years of experience and training in services to elderly or adults with disabilities. Have at least two year’s work experience in supervision and administration. Have a written medical statement, completed within the past 12 months by a physician, nurse practitioner or physician’s assistant, certifying that the employee has no illness or health condition that would pose a risk to others and that the employee can perform the duties assigned in the job and has a clean drug test. If not available, this can be scheduled and paid for by the Agency. Pass a Criminal background Check, also paid for by the Agency.
Provide at least three references from individuals with whom a reference interview can be conducted, including at least one former employer. The individuals providing reference information shall have knowledge of the applicant’s background and qualifications.
This is a full time, permanent, salaried, non-exempt position: Salary is currently $28,080 plus, per year based on Education/Experience.
